What Does a Semen Analysis Tell You About Male Fertility?
A semen analysis is one of the most important initial tests used to evaluate male fertility. It provides information about the sperm contained in a semen sample and can help identify abnormalities that may affect the chances of conception.
However, a semen analysis is more than simply checking whether a man's sperm count is "normal."
The test can provide information about several important characteristics of sperm, including their number, movement and shape. When interpreted alongside a man's medical history, physical examination and the fertility history of both partners, these results can help identify possible causes of reduced fertility.

What Is a Semen Analysis?
A semen analysis is a laboratory examination of an ejaculated semen sample.
The test evaluates several characteristics that can provide information about male reproductive potential.
Depending on the laboratory and the clinical situation, the assessment may include:
Semen volume
Sperm concentration
Total sperm number
Sperm motility
Sperm morphology
Sperm vitality
Presence of other cells or abnormalities
These parameters should not be considered independently.
For example, a man may have a relatively good sperm concentration but reduced motility. Another man may have an adequate number of sperm but a significant reduction in normal morphology.
The overall interpretation therefore depends on the combination of findings.
What Does a Semen Analysis Measure?
1. Semen Volume
Semen volume refers to the amount of semen produced during ejaculation.
A low volume can sometimes be associated with problems involving the reproductive tract, hormonal factors or incomplete collection of the sample.
However, semen volume alone cannot diagnose infertility.
It needs to be interpreted together with the other findings.
2. Sperm Concentration
Sperm concentration refers to the number of sperm present in a specific volume of semen.
A lower-than-expected concentration is commonly referred to as oligozoospermia.
Low sperm concentration can have many possible causes, including:
Varicocele
Hormonal abnormalities
Testicular disorders
Genetic conditions
Previous infections
Certain medications
Testosterone or anabolic steroid use
Environmental or lifestyle factors
This is why discovering a low sperm count is only the beginning of the evaluation.
The next question is:
Why is the sperm count low?
3. Total Sperm Number
Total sperm number refers to the total number of sperm present in the entire ejaculate.
This is different from sperm concentration.
For example, two semen samples may have the same sperm concentration but different semen volumes, resulting in different total sperm numbers.
Both measurements can therefore provide useful information when assessing male reproductive potential.
4. Sperm Motility
Sperm motility describes how sperm move.
Movement is important because sperm need to travel through the female reproductive tract to reach and fertilise an egg.
Laboratories may distinguish between sperm that move progressively and those that show other types of movement.
Reduced sperm movement is called asthenozoospermia.
Low motility may occur alongside other sperm abnormalities and can have several possible causes.
It is therefore important not to interpret motility in isolation.
5. Sperm Morphology
Sperm morphology refers to the shape and structure of sperm.
A semen analysis may assess the proportion of sperm that have a morphology considered normal according to established laboratory criteria.
Abnormal morphology does not automatically mean that pregnancy is impossible.
Sperm morphology is one parameter among several, and its significance depends on the overall semen analysis and clinical context.
6. Sperm Vitality
Sperm vitality assesses whether sperm are alive.
This can be particularly useful when sperm motility is very low.
A sample may contain sperm that are alive but not moving normally, while other sperm may be non-viable.
Vitality can therefore provide additional information when interpreting severely abnormal motility.
What Do Normal Semen Analysis Results Mean?
One of the most important things to understand is that normal semen analysis results do not guarantee fertility.
Likewise, an abnormal result does not automatically mean that natural conception is impossible.
Semen analysis provides information about sperm characteristics, but fertility depends on many factors involving both partners.

What Are Reference Values in a Semen Analysis?
Laboratories compare semen results with established reference values.
These values can help identify whether a parameter is below, within or above a reference range.
However, reference values should not be interpreted as a strict boundary between fertility and infertility.
For example:
A sperm count below a reference value does not mean that conception cannot occur.
And:
A sperm count within the reference range does not guarantee that conception will occur.
This distinction is extremely important when interpreting a semen analysis.
What If the Semen Analysis Is Abnormal?
An abnormal result does not necessarily mean that something is permanently wrong.
Semen parameters can vary between samples.
Factors such as illness, fever, medication, lifestyle changes and differences in the collection process can influence results.
For this reason, if the first semen analysis is abnormal, a repeat analysis may be recommended.

What Can Cause an Abnormal Semen Analysis?
There are many possible causes.
Varicocele
An enlarged vein around the testicle can be associated with impaired sperm production in some men.

Hormonal Problems
Hormonal disorders can interfere with the signals responsible for sperm production.

Testosterone or Anabolic Steroid Use
External testosterone can suppress the body's natural reproductive hormones and reduce sperm production.
This is particularly important for men who are taking testosterone while trying to conceive.

Testicular Problems
Conditions affecting the testicles can interfere directly with sperm production.

What If No Sperm Are Found?
When a semen analysis repeatedly shows no sperm, the condition is called azoospermia.
This does not necessarily mean that sperm cannot be produced.
The next step is to determine whether the azoospermia is:
Obstructive
Sperm production may be occurring, but sperm are prevented from reaching the semen because of an obstruction.
Non-obstructive
Sperm production itself is significantly impaired.
Further investigations may include hormonal testing, genetic assessment, physical examination and other specialised investigations.

Can a Semen Analysis Detect Sperm DNA Damage?
Not directly.
A standard semen analysis primarily evaluates conventional parameters such as:
Number
Concentration
Movement
Morphology
Vitality
It does not directly measure the integrity of sperm DNA.
In selected patients, a sperm DNA fragmentation test may provide additional information.
This may be considered in situations such as:
Unexplained infertility
Recurrent pregnancy loss
Repeated assisted reproductive treatment failure
Selected cases of otherwise unexplained reproductive problems

However, sperm DNA fragmentation testing is not a routine replacement for semen analysis.
How Should You Prepare for a Semen Analysis?
Your laboratory or fertility specialist should provide specific instructions before the test.
These may include guidance regarding:
Period of sexual abstinence
Collection of the entire sample
Use of an appropriate sterile container
Avoiding contamination
Keeping the sample within the recommended temperature range
Delivering the sample to the laboratory within the specified time
Following the laboratory's instructions carefully is important because incorrect collection or handling can affect the reliability of the results.
If any part of the sample is lost during collection, tell the laboratory rather than assuming the result will be unaffected.
Does Ejaculation Frequency Affect Sperm Quality?
Sperm production is continuous, but the characteristics of a semen sample can vary depending on the time since the previous ejaculation.
This is one reason laboratories provide specific instructions regarding abstinence before testing.
The goal is to obtain a sample under standardised conditions so that the results can be interpreted appropriately.
If repeat testing is required, following similar preparation instructions can make comparison between samples more useful.
What Happens After a Semen Analysis?
The result should not be viewed in isolation.
A specialist may combine the semen analysis with:

An abnormal semen analysis does not automatically mean that fatherhood is impossible.

The duration of infertility
The female partner's fertility
The couple's age and reproductive circumstances
Whether a treatable cause can be identified
Some men may conceive naturally after addressing an underlying problem, while others may benefit from assisted reproductive techniques.
The important step is understanding why the semen analysis is abnormal.
When Should You Speak to a Male Fertility Specialist?
Consider a specialist assessment if:
Your semen analysis is abnormal.
You have been told that your sperm count is low.
No sperm were found in your semen.
You have a known varicocele.
You have a history of testicular problems.
You have previously used testosterone or anabolic steroids.
You have been trying to conceive without success.
