What Is a Semen Analysis?


A semen analysis is a laboratory test that examines a man's semen and sperm. It measures several factors that affect fertility, such as sperm count, movement, shape, and the overall quality of the semen. Doctors often recommend this test when a couple has trouble conceiving or when a man wants to check his fertility status.


The test involves collecting a semen sample, usually through masturbation, which is then analyzed in a lab. The results provide important clues about male reproductive health.


Why Is a Semen Analysis Important?


Semen analysis helps identify potential causes of infertility. Male factors contribute to about 40-50% of infertility cases, so this test is often the first step in understanding fertility challenges. It can also detect infections or other health issues that might affect sperm production or function.


Knowing your semen analysis results can guide treatment decisions, lifestyle changes, or further testing. It also helps set realistic expectations for conception.


WHO Reference Values for Semen Analysis


The World Health Organization (WHO) provides standard reference values to help interpret semen analysis results. These values represent the lower limits of normal based on healthy men whose partners conceived within a year. Here are some key reference points:


  • Sperm concentration: 15 million sperm per milliliter (mL) or more

  • Total sperm count: 39 million sperm per ejaculate or more

  • Motility: 40% or more of sperm moving, with at least 32% moving progressively

  • Morphology: 4% or more of sperm with normal shape

  • Semen volume: 1.5 mL or more

  • pH: 7.2 to 8.0

  • White blood cells: Less than 1 million per mL


Values below these thresholds may indicate fertility issues, but they do not guarantee infertility. Many men with values outside the normal range still conceive naturally.


Understanding Key Semen Analysis Components


Sperm Count


Sperm count measures how many sperm are present in one milliliter of semen. A higher count generally increases the chance of fertilizing an egg. Counts below 15 million per mL are considered low (oligospermia). Zero sperm in the sample is called azoospermia and requires further investigation.


Example: If your sperm count is 10 million per mL, this is below the WHO reference. Your doctor might suggest lifestyle changes or further tests to find the cause.


Motility


Motility refers to how well sperm move. Healthy sperm swim forward in a straight line or in large circles. The test reports the percentage of moving sperm and the percentage moving progressively (forward). Low motility (asthenozoospermia) can reduce the chance of sperm reaching the egg.


Example: If only 20% of your sperm move forward, this is below the normal 32% threshold and may affect fertility.


Morphology


Morphology looks at the shape and structure of sperm. Normal sperm have an oval head and a long tail. Abnormal shapes can affect the sperm’s ability to fertilize an egg. The WHO considers 4% or more normal forms as acceptable.


Example: If only 2% of your sperm have a normal shape, this is low morphology (teratozoospermia), which might require further evaluation.


Semen Volume


This measures how much semen is ejaculated. A volume under 1.5 mL is considered low (hypospermia). Low volume can result from issues with the seminal vesicles or prostate gland.


Example: A volume of 1 mL might suggest a blockage or other problem affecting semen production.


pH Level


Semen pH indicates how acidic or alkaline the fluid is. Normal semen is slightly alkaline, with a pH between 7.2 and 8.0. A lower pH may suggest infection or blockage in the reproductive tract.


White Blood Cells


The presence of white blood cells in semen can indicate infection or inflammation. More than 1 million white blood cells per mL is abnormal and may require treatment.


Common Abnormal Results and What They Mean


  • Low sperm count (oligospermia): May result from hormonal imbalances, varicocele, infections, or lifestyle factors like smoking.

  • No sperm (azoospermia): Could be due to blockage, genetic issues, or testicular failure.

  • Poor motility (asthenozoospermia): Can be caused by infections, heat exposure, or oxidative stress.

  • Abnormal morphology (teratozoospermia): Often linked to genetic factors or environmental toxins.

  • Low semen volume: May indicate ejaculatory duct obstruction or hormonal problems.

  • High white blood cells: Suggests infection needing antibiotics or further evaluation.


Each abnormal result has different implications and treatment options. Your doctor will help interpret your report in the context of your overall health and fertility goals.


When Should You Repeat the Test?


Semen analysis results can vary due to illness, stress, or recent ejaculation. If your first test shows abnormal results, your doctor usually recommends repeating the test after 2 to 3 months. This allows time for sperm production cycles to reset and helps confirm whether the findings are consistent.


Avoid ejaculation for 2 to 7 days before the test to get accurate results. Also, try to reduce stress and avoid alcohol or heat exposure before testing.


When Should You See a Fertility Specialist?


If repeated semen analyses show abnormal results, or if you have other fertility concerns, seeing a fertility specialist is a good next step. A specialist can perform more detailed tests, such as hormone levels or genetic screening, and discuss treatment options like medication, surgery, or assisted reproductive technologies.

  • A history of testicular injury or surgery

  • Signs of infection or inflammation

  • Difficulty conceiving after one year of regular unprotected sex
