Semen Analysis: How to Understand Your Male Fertility Test
A semen analysis is one of the most important initial tests used to assess male fertility.

Why Is a Semen Analysis Important?
Male factors contribute to a substantial proportion of couples experiencing difficulty conceiving.
A semen analysis provides an important starting point because it can identify abnormalities that may otherwise have no obvious symptoms.

Normal libido
Normal erections
Normal testosterone
No pain
No obvious testicular abnormality
and still have a significantly abnormal semen analysis.
This is why fertility cannot be assessed from symptoms alone.
What Does a Semen Analysis Measure?

Semen Volume
The amount of fluid produced during ejaculation.
Sperm Concentration
The number of sperm present per millilitre of semen.
Total Sperm Number
The total number of sperm in the entire ejaculate.
Motility

Each parameter provides different information.
What Is Sperm Concentration?
Sperm concentration refers to the number of sperm present in a specific volume of semen.
A low concentration is known as oligozoospermia or oligospermia.
The severity can vary considerably.
A man may have:
Mildly reduced sperm concentration
Moderately reduced concentration
Severely reduced concentration
No detectable sperm
The last condition is called azoospermia.
What Is Total Sperm Number?
Total sperm number is different from sperm concentration.
For example, a sample may have a relatively good sperm concentration but a low total sperm number if the semen volume is very small.
Conversely, a lower concentration may still result in a reasonable total sperm number if the semen volume is larger.
This is why the complete semen analysis is more informative than looking at a single number.
What Is Sperm Motility?
Sperm motility refers to the ability of sperm to move.
Movement is important because sperm need to travel through the female reproductive tract and eventually reach the egg.
Motility is generally assessed by dividing sperm into categories based on their movement.
A reduced proportion of moving sperm may be described as asthenozoospermia.
What Is Progressive Motility?
Progressive motility refers to sperm that move forward rather than simply moving in place.
Forward movement is particularly relevant because sperm need to travel through the reproductive tract.
A semen analysis therefore does not simply ask whether sperm move.

For this reason, an abnormal result may need to be confirmed with another semen analysis.

How Should You Prepare for a Semen Analysis?
The laboratory will provide specific instructions.
Generally, the recommended abstinence period is limited to a few days before the sample.
It is important to follow the laboratory's instructions carefully because collection and handling can affect the result.
The sample should usually be collected into the sterile container supplied or approved by the laboratory.
Does the Entire Ejaculate Need to Be Collected?
Yes.
The first portion of the ejaculate can contain a particularly high concentration of sperm.
Losing part of the sample may therefore affect the accuracy of the test.
If any portion of the sample is lost during collection, the laboratory should be informed.
How Is a Semen Sample Collected?
The sample is usually collected by masturbation into a sterile laboratory container.
Some men may find the process uncomfortable or stressful.
However, accurate collection is important because incomplete collection can affect the interpretation of the results.
If there are difficulties with sample collection, the fertility clinic should be informed rather than attempting to alter the procedure independently.
What Can Cause a Low Sperm Count?
A low sperm count can have many possible causes.

What Does a Low Motility Result Mean?

When Should Hormone Testing Be Added?

Depending on the diagnosis, treatment may include hormonal therapy, surgical correction or surgical sperm retrieval such as micro-TESE.
Can Semen Analysis Predict Natural Pregnancy?
Not perfectly.
Semen analysis provides important information, but it is not a complete fertility test.
Pregnancy depends on both partners and multiple biological factors.
A man with borderline semen parameters may father a child naturally, while a man with apparently reasonable parameters may still experience infertility.
The semen analysis should therefore be viewed as one component of the fertility evaluation.
How Often Should a Semen Analysis Be Repeated?
There is no single schedule that applies to every man.
Repeat testing may be recommended when:
The first result is abnormal
The sample collection was incomplete
A temporary illness may have affected the result
Treatment has been started
