Hormonal Causes of Male Infertility: How Hormones Affect Sperm Production

This is different from the testosterone level measured in a standard blood test.
Therefore, increasing blood testosterone with external testosterone does not necessarily increase sperm production.
In fact, external testosterone can suppress the hormonal signals required for spermatogenesis.
Can Testosterone Treatment Cause Infertility?
Yes.

Taking exogenous testosterone can suppress GnRH, FSH and LH.
As a result, stimulation of the testicles decreases and sperm production can fall dramatically.
In some men, sperm production can stop altogether.

What Is FSH and Why Is It Important?
Follicle-stimulating hormone (FSH) is produced by the pituitary gland.
It plays an important role in supporting sperm production.
FSH is therefore often measured when evaluating severe male infertility.
An elevated FSH level can indicate that the testicles are not producing sperm effectively.
However, FSH alone cannot determine whether sperm are completely absent.
It must be interpreted alongside:
Semen analysis
Testicular volume
Testosterone
LH
Medical history
Physical examination
What Does High FSH Mean in Men?
A high FSH level can be associated with impaired sperm production.
It may suggest that the testicles are receiving strong hormonal stimulation but are not responding adequately.
This can occur in conditions affecting the testicular tissue itself, including some forms of non-obstructive azoospermia.
However, a high FSH level does not automatically mean that sperm retrieval is impossible.

Hyperprolactinaemia means that the level of prolactin in the blood is elevated.
High prolactin can interfere with normal reproductive hormone signalling.
