MALE FERTILITY SPECIALIST · LONDON
Told IVF or donor sperm is your only option? Let's look at the male side properly.
A semen test tells you what is happening, not why. Professor Amr Raheem, Consultant Andrologist, looks for the cause and treats it where he can, to help you conceive naturally or give IVF its best chance.

Which of these sounds like you?

I've been told I have no sperm
It doesn't always mean you can't have your own biological child. Sperm can often still be found, by treating a blockage or with microsurgical sperm retrieval.
My sperm count is low
A low count is a finding, not a diagnosis. Many causes can be treated, so you aren't sent straight to IVF.
My testosterone is low, or I've been told I have a hormone problem
Hormone problems are among the most treatable causes of male infertility, but testosterone alone switches sperm production off. There are better options.
IVF hasn't worked for us
Sperm quality, DNA damage and hormones can be overlooked between cycles. We look for what can still be improved before the next attempt.
I've used testosterone or steroids
Sperm production can often be restarted. Treatment is tailored so you can stay well while your fertility recovers. No judgement.
We'd like to try naturally
Treating causes such as a varicocele, an infection or a hormone imbalance can give you a real chance of conceiving without IVF.
I'm about to start cancer treatment
We can arrange sperm freezing quickly, before treatment begins, including when producing a sample isn't possible.
I've had a vasectomy and want children
Microsurgical reversal can restore sperm in the ejaculate. Where it isn't suitable, sperm can be retrieved for IVF.
My partner has had miscarriages
Sperm can carry DNA damage that a normal semen test doesn't show. It can be measured, and often reduced.
Our tests are normal, but we're still not pregnant
A “normal” semen test doesn't rule out a male factor. A specialist assessment often finds something that can be improved.
Not every varicocele needs treatment, but repairing the right one can improve sperm and the chance of natural pregnancy.

My sperm don't move well, or are an abnormal shape
Poor motility and morphology often have a cause, such as a varicocele, infection or hormone imbalance, that can be treated.
DNA damage can lower the chance of pregnancy and raise the risk of miscarriage. Finding and treating the cause can often reduce it within about three months.

I'm over 40 and want to become a father
The male biological clock is real. Sperm quality and DNA decline with age, but much of it can be measured and improved.

For men with no sperm in the semen, sperm can be retrieved on the same day as your partner's egg collection and used fresh, without being frozen and thawed. This matters most when very few sperm are found.

For men with a good sperm count but high DNA fragmentation, sperm taken directly from the testis usually carries two to three times less DNA damage than ejaculated sperm. It is used fresh for ICSI on the day of egg collection.
